Currently seeking a Case Manager, RN, to work in our Case Management Department in Flower Mound, Texas, and who will provide service excellence and support the hospital's mission, vision and values through appropriate conduct and demeanor at all times.
Administers all aspects of case management to include:
Conducts screening and assessment activities and collaborates with the healthcare team for the purpose of identifying continuum of care needs. Works closely and effectively with other members of the healthcare team to include the preferences of the patient in developing a patient centered transitional plan of care. Assists patients as they transition to the next level of care to include medication procurement, SNF, DME, home health, hospice referral, etc. and negotiates with service providers, as indicated to avoid delays in discharge. Communicates payer issues to the patient and healthcare team and strives to avoid readmissions to the hospital setting. Serves as a patient and facility advocate and performs utilization review activities, as indicated.
The ideal candidate would have:
Cognitive, technical and communication skills effective in fostering a team environment that included the patient, the family, the physician and other members of the healthcare team. The ability to create an environment to support patient safety, and be able to advocate on the behalf of the patient and/or the facility for needed client resources and services. The ability to be flexible, to work Monday-Friday with a minimal shared rotation of holidays and weekends.
-Current TX RN license required
-Associate/Bachelor degree required-
-Current case management experience or 5 years recent hospital experience on a Med/Surg unit. Case Management Certification preferred.
-Experience with InterQual or Milliman admission criteria a plus.
